For best results for lower extremity, e.g., foot, ankle, i recommend you PSW, the pnuematic shockwave system especially for bones and joint.

The biggest head (photo as below) you mentioned is newly added and mainly used for shallow tissue layer, it’s better for skin treatment than working in tendons or bone, and patients usually feel obvious pain while using this head. The other 11 black heads penetrate deeper and patients commonly feel comfortable.
